You’re at least 18 years of age or older
The last driver’s license you carried was obtained in person at a DMV office
You haven’t had a name change
In addition to your full name as it appears on your license, you will need to provide your driver’s license number, your Social Security number, and your date of birth.
You must also certify that your vision is 20/40 or better, you haven’t changed your name, and your address is correct. If you’ve moved, update your address online
before proceeding with the replacement process. Your request should be processed within three to five business days, and you will receive an emailed receipt and a copy of your temporary license then. Print out this temporary license and carry it with you until you receive your duplicate license arrives, which should take between two to four weeks by mail.
